NY Connects gives information and counseling to people of all ages who need long term services and supports to help them maintain or improve their daily lives and health. This could be an older adult, a child or an adult with a disability, a family member or caregiver, a friend or a neighbor, or even a helping professional. NY Connects is a resource that links the community to services of their choice in order to remain independent as long as possible regardless of insurance.

Information and Assistance is provided by telephone, presentations, Dispatch articles, the Delaware County NY Connects Website, and by appointment. NY Connects partners with key area resources and services through the Long Term Care Advisory Council and collaborative projects to improve communication and cooperation among service providers and caregivers. Since 2006, Delaware County NY Connects and the Long Term Care Advisory Council have looked at gaps in services and the challenges we face in Delaware County.
